The CPAA Care Coordination project is requesting information from organizations in the region that already deliver care coordination services. The survey is open through February 23rd and takes about 20 minutes to complete.
The data collected will be used to make decisions about target population(s), identify criteria for selecting care coordinating agencies for the project, and to quantify gaps between existing care coordination capacity and needed workforce and technology investments to reach implementation goals.
“We’ve had a great response so far,” said Michael O’Neill, Pathways Program Manager for CHOICE Regional Health Network. “This information will give us what we need to design an implementation that is right for our region.” He encourages anyone working on care coordination to participate in the survey.
